A woman aged 91 froze to death outside her care home after staff failed to notice she was missing for nearly 12 hours, a court heard.

Prosecutor Bernard Thorogood said Hilda Fairweather had dementia and was known to wander off.

Yet no head count was taken of residents after a fire door was found ajar and no checks were made on her in the night.

He added: “The sad facts are she froze to death when she should not have been able to get out.”

The court heard the 29 residents were being cared for by just two staff.

Some had also wandered off before, with one found in a ditch.

Abele View Ltd, which ran the home near Stourbridge, West Mids, was fined £133,000 at Stoke court after admitting failing to ensure the safety of its residents.
